Who we are

When we say "we", "us" or "our(s)", we are referring to Donna Rumble Professional Corporation, operating as Supreme Caregiver Support™ and Supreme Foot Care™, or ("the corporation"). We are not an agency, nor are we contracted by any organization to provide healthcare services. We are an independent professional nursing practice, federally incorporated and registered for business in the Province of Ontario. Supreme Caregiver Support organization is a nurse-led professional nursing practice that offers private nursing services and supports to caregivers and families. We do not provide emergency medical services. If you have a medical emergency, please call 911 or go to your nearest emergency department.

Our Privacy Statement

Supreme Caregiver Support is committed to respecting your privacy and protecting your personal information. This policy pertains to this website and the collection and use of your personal information. Our privacy policy is updated as needed to maintain our commitment to government rules and regulations.

The need for Privacy

Pursuant to the Personal Health Information Protection Act, 2004 (PHIPA), Personal Health Information is information that governs health care information privacy in Ontario. Information privacy is defined as the client's right to control how his/her personal health information is collected, used and disclosed. PHIPA sets consistent rules for the management of personal health information and outlines the client's rights regarding their personal health information. This legislation balances a client's right to privacy with the need of individuals and organizations providing health care to access and share health information. PHIPA permits the sharing of personal health information among health care team members to facilitate efficient and effective care. The health care team includes all those providing care to the client, regardless of whether they are employed by the same organization. PHIPA requires the health information be kept confidential and secure.

Declaration

We may only use personal information for the purpose it has been collected, or for a consistent purpose, or if another use is authorized by law. We may only disclose personal information when legally obligated to do so, or for the sole purpose of providing you with the service(s) you have consented to.

How we use your information

We do not collect any personal information on this website. If you choose to inquire or use a service we offer, you will be required to provide your email address and/or telephone number when you contact us. Your information will not be shared with anyone without your consent, nor will we sell your contact information to a third party. You will also be required to fill out a form, which will be emailed to you upon your request. This form, a product of Google ("Google Forms") adheres to the same robust privacy commitments and data protections as the rest of Google Cloud's Enterprise Services, as stated by Google. The form will consist of several required and optional questions about your request to determine how best to assist you. If you wish to continue with our service, an in-home consultation will be provided to you at a cost. All personal and health information obtained during this consultation will be handled in accordance with the Personal Health Information Protection Act, 2004. We will never ask you to submit personal information on the social media platforms we manage. Your information will either be obtained through email using Secured Sockets Layer technology or by an in-person visit to your place of residence, or both.

Digital Software

When you book an appointment with us using Koalendar appointment scheduling software, you will be asked to provide your name, email address and phone number. Supreme Caregiver Support only uses your information to contact you and send you our service form, at your request. If you are not comfortable using this software, call or email us for further assistance.

Links to other websites and social media platforms

Social media platforms we manage may provide a link to our websites as a convenience or courtesy to the user(s). They are not associated with us, endorsed by us, or bound by our privacy statement. You should review the privacy policies of any linked website or social media platform before providing any personal information. 

When you contact us about becoming a PEER, your information will only be used to provide you with further details about our program.
