Nurse-Led Disability Care

DSUPREME®  provides nurse-led care and supportive services for children and adults living with developmental disabilities and complex medical needs. 

Services are delivered through individualized nursing assessment and care planning to promote safety, participation, and continuity of care across environments.

Support may be provided:

• In the home

• In school settings

• During community activities and outdoor events

• At day programs and camps

• During structured retreats or organized programming

Services May Include:

• Health monitoring and clinical oversight

• Medication support within nursing scope

• Assistance with Activities of daily living

• Behavioural support coordination (as appropriate)

• Communication and coordination with families and interdisciplinary teams

• Support to promote inclusion and safe participation

All services are delivered within defined nursing scope and professional standards. A consultation and Service Agreement are required prior to initiating care.
