NURSE-LED MEDICAL FOOT CARE
DSUPREME® provides nurse-led medical foot care services delivered in-home through professional nursing assessment, clinical judgment, and evidenced-informed practice. Guided by the Live Well at Home™ Philosophy, services focus on preventive care, risk identification, and ongoing monitoring to support comfort, mobility, independence, and overall foot health.
WHO MAY BENEFIT FROM NURSING FOOT CARE
Nursing Foot Care may be appropriate for individuals who:
Live with diabetes, circulatory conditions, or other chronic health concerns
Experience reduce mobility or difficulty performing foot self-care
Have thickened, fungal, in-grown, or difficult-to-manage nails
Are at risk of skin breakdown, infection, or foot complications
Require ongoing monitoring due to chronic health conditions
Prefer professional in-home care for safety, comfort and convenience
THE IMPORTANCE OF PROPER NAIL CARE
Did you know that improper nail trimming techniques can increase the risk of infection, particularly for individuals living with diabetes,neuropathy, or circulatory conditions?
The cuticle and hyponychium are two important membranes of the nail unit. The cuticle forms a seal at the base of the nail plate, helping to prevent bacteria and pathogens from entering the nail unit. The hyponychium, located beneath the free edge of the nail, provides protection to the underlying nail bed.
Cutting or disrupting these protective tissues can create an entry point for infection. Nursing foot care emphasizes safe technique, sterile instrumentation, and clinical assessment to preserve tissue integrity and reduce the risk of preventable complications.
WHAT IS INCLUDED
Assessment of skin integrity, circulation, and sensation
Nail trimming and nail management
Reduction of calluses and thickened skin
monitoring for early signs of ulceration, infection, or other foot complications
Education regarding foot hygiene, footwear, preventative foot care
Referral recommendations when concerns require physician, nurse practitioner, podiatry, chiropody, or other specialist evaluation
HIGH -RISK FOOT CARE
Individuals living with diabetes, neuropathy, vascular conditions, impaired healing, or other chronic health concerns may require high-risk foot care. Nursing assessment focuses on early identification of complications and ongoing monitoring to reduce the risk of preventable injury, infection, ulceration, and lower-extremity complications.
Through regular assessment and evidence-informed foot care, potential concerns may be identified early and referred fofr further medical evaluation when appropriate.
MOBILE / IN-HOME DELIVERY
In-Home Mobile Foot Care
Services are delivered in the home to promote safety, comfort, and accessibility. In-home nursing foot care may be particularly appropriate for individuals with limited mobility, transportation barriers, chronic health conditions, or complex medical needs. Mobile service delivery allows clients to receive professional foot care in a familiar and convenient environment.
CONSULTATION & FEES
Consultation & Service Agreement
A consultation is required prior to initiating services. A service agreement is completed to clearly outline the scope of care, responsibilities, and applicable fees associated with the services provided.
Fees
Foot care services are billed at a based rate. Additional charges may apply for specialized tools, supplies, travel outside designated service areas, and applicable taxes.
