SERVICES
WHAT WE OFFER
The Ontario government coordinates healthcare services through Home and Community Care Support Services. Services are provided for people of all ages who require care in their homes, at schools or in the community. To find out if you qualify for government funding, contact your Local Home and Community Care Support Services organization. If you do not qualify for government funding, Supreme Caregiver Support™ can provide private services to you at a cost. These costs may be covered by private health insurance plans, or claimed as an out-of-pocket health expense on your income tax. Some caregivers may also receive special funding through government programs to hire their own private caregiver.
Specialty services are end-of-life care, disability care, foot care, wound care, and other services that fall under their categories. These services require more time and are often accompanied by complex issues. There is more time set aside for the client who requires one or more of these services.
A consultation is required prior to accessing services. A Service Agreement will outline the care that is to be provided.
Fees are affordable and service is guaranteed.
For foot care, fees are assessed at a base rate plus the price of tools and taxes. The base rate may range from $75 to $95 per visit, depending on the physical assessment.
Services for home care, end-of-life care and disability care are billed hourly or in blocks of time. Fees may vary depending on the service(s) required.
All assessment fees are billed separately and do not reflect the actually cost of a specific service.
