Nurse-Led Home Care

DSUPREME®  provides nurse-led in-home care services designed to support health, independence, mobility, and quality of life in the home and community.

Services are tailored to the unique needs of each individual and may be delivered in private homes, retirement residences, long-term care settings, or other community environments.

Through professional nursing assessment, care planning, care coordination, and ongoing monitoring, we help individuals and families access the support the need to Live Well at Home™.

Services may include:

• In-home nursing assessments and care coordination

• Medication management and monitoring

• Wound assessment and dressing changes

• Post-injury or post-illness recovery support

• Monitoring of health status and chronic conditions

• Assistance with hygiene, including bathing, oral care, skin care, and foot care

• Support with eating, hydration, toileting, dressing, and mobility

• Companion support within a care plan

• Accompaniment to and from medical appointments, when appropriate

All services are provided within defined nursing scope and professional standards. A consultation and Service Agreement are required prior to initiating care.
