Nurse-Led End-of-Life Care

DSUPREME®  provides nurse-led end-of-life and palliative care support for individuals who wish to remain at home.

Palliative care focuses on comfort, dignity, and quality of life for individuals living with serious, progressive, or life-limiting illnesses. This may include conditions such as advanced cancer, heart failure, lung disease, neurological conditions including dementia, and other complex health challenges.

Services are delivered through professional nursing assessment and individualized care planning to support both the client and their caregivers.

Support May Include:

• Symptom monitoring and comfort-focused care

• Pain management support within nursing scope

• Medication oversight and coordination

• Assistance with personal care and hygiene

• Respite support for caregivers

• Overnight monitoring when appropriate

• Care coordination and communication with the broader healthcare team

In Ontario, publicly-funded home and community care programs may provide palliative support based on eligibility criteria. Availability and hours of service vary by region and program structure.

Some families choose to supplement publicly-funded services with private nursing care to enhance continuity, flexibility, or additional support.

A consultation and Service Agreement are required prior to initiating services.
