END-OF-LIFE CARE
Nurse-Led End-of-Life Care
DSUPREME® provides nurse-led end-of-life and palliative care support for individuals who wish to remain at home.
Palliative care focuses on comfort, dignity, and quality of life for individuals living with serious, progressive, or life-limiting illnesses. This may include conditions such as advanced cancer, heart failure, lung disease, neurological conditions including dementia, and other complex health challenges.
The Live Well at Home™ Philosophy recognizes that living well is not defined by the absence of illness, but by the presence of comfort, dignity, meaningful relationships, and care that reflects an individual's goals and values. Through nurse-led palliative care, we support individuals and families by focusing on quality of life, symptom management, informed decision-making, and compassionate support throughout the care journey.
Services are delivered through professional nursing assessment and individualized care planning to support both the client and their caregivers, guided by this philosophy.
Support May Include:
Symptom monitoring and comfort-focused care
Pain management support within nursing scope
Medication oversight and coordination
Assistance with personal care and hygiene
Respite support for caregivers
Overnight monitoring when appropriate
Care coordination and communication with the broader healthcare team
In Ontario, publicly-funded home and community care programs may provide palliative support based on eligibility criteria. Availability and hours of service vary by region and program structure.
Some families choose to supplement publicly-funded services with private nursing care to enhance continuity, flexibility, or additional support.
A consultation and Service Agreement are required prior to initiating services.
