Feet Fit For Life™ is a nurse-developed foot health initiative focused on screening, education, and preventive care to support long-term mobility.
An Annual Foot Health Program Supporting Mobility & Early Intervention
Feet Fit For Life™ Program is DSUPREME®'s annual foot health program designed to support mobility, comfort, and overall foot health through proactive nursing care. The program focuses on early identification of foot health concerns, preventive intervention, and ongoing support to help reduce complications that can impact balance, mobility, and independence.
Through nurse-led assessment and evidenced-based foot care, the program emphasizes timely intervention, education, and continuing care tailored to individual needs. Services are delivered with a strong focus on safety, dignity, and functional outcomes, supporting clients in maintaining mobility and quality of life throughout the year.
Feet Fit For Life™ reflects our commitment to preventative care and early action addressing foot health concerns before they progress and supporting long-term mobility at every stage of life.
Our basic foot care includes trimming of normal nails without infection, and with no impaired sensation or swelling of the feet. Our advanced foot care includes the reduction of corns, calluses, and thickened and malformed toenails; removal of ingrown toenails and plantar warts, and treatment of blisters and heel fissures.
When higher-risk foot concerns are identified, referrals are made to appropriate health professionals, including Chiropodists or Podiatrists, as well as medical doctors or nurse practitioners, for further assessment, diagnosis, and medication management as required.
CARE COORDINATION & SYSTEM NAVIGATION
When care needs extend beyond the scope of private or short-term nursing services, appropriate referrals are made to publicly funded home and community care programs, including Ontario Health atHome, to support continuity, safety, and access.
Activities of daily living (ADLs) support and coordination
Community support services and resource navigation
Home safety assessment and fall risk considerations
Mobility support and fall prevention planning
Support related to social isolation and psychosocial needs
Behavioural support considerations and appropriate referral pathways
Dementia and memory impairment support, education, and coordination
Housing options navigation and care planning support
Pain assessment, system support, and coordination of care
Strengthening and chronic disease management support
Medication review support and coordination with prescribing providers
Palliative and end-of-life care coordination and family support
Referral and coordination with allied health services, including speech-language pathology
